• Abstract: General practice medicine includes community health care, community preventive medicine, and community health management and so on. It is more complete in community medical care and community vaccination medicine. But in the aspect of community healthy management, there are a lot of deficiencies that restrict the role of general practice medicine in the health management. Changling Community Healthy Center of Shenzhen Luohu Hospital Group take advantage of Graded public health service package to observe community health management through general practice team construction, refine the division of labor and collaboration of team members. Community center utilize operable community healthy manage skill and implement community health management to reshape the healthy lifestyle of residents. Fundamentally transform Disease-centered to Healthcentered; transform biological model to patient-oriented. 全科医学，包含社区医疗、社区预防医学、社区健康管理等方面。在 社区医疗、社区预防接种医学上，比较完善。在社区健康管理方面，存在较大 不足，制约着全科医学在健康管理上的作用。深圳罗湖医院集团长岭社区健康管理中心，通过全科医生团队建设，细化团队成员分工及协作，利用分级公共 卫生服务包，对社区居民进行“社区健康管理”研究，利用可操作的社区健康 管理技能、让社区健康管理落地，重塑居民健康生活方式。从根本上实现以疾 病为核心向以健康为核心的转移；以生物模式为主，向以人为本的转移。
