孤独雏鸟的生存战役 ——一例基于心理动力学视角与双相情感障碍来访者工作的案例报告

• 摘要： 本案例来访者主动咨询，主诉是情绪低落无动力，轻生意念；自责自罪、抑郁、焦虑情绪明显。精 神科诊断为双相II 型，抑郁发作中；咨询师基于动力学人格评估，从自我心理学，客体关系，自体心理学 以及心智化理论等角度来理解来访者。咨询主要运用心理动力学理论，在理解来访者的基础上，建立共情 但有边界的关系，促进心智化；运用自由联想和移情诠释等技术增加来访者的觉察与反思，并且稳定地陪 伴来访者，促进共生、分离-个体化完成。目前来访者能够更好地理解过去是怎样绑架当下的感受，能够更 有边界地维系关系，并且能够更好地理解并接纳自己。
• In this case, the client took the initiative to consult and complained that he was depressed and had no motivation, Suicidal ideation. Self-accusation, depression and anxiety are obvious. Psychiatric diagnosis as bipolar II, depression attack; Based on the dynamic personality assessment, the consultant understands the client from the perspectives of ego psychology, object relationship psychology, self psychology, and mentalization theory. Counseling mainly used psychodynamic theory to establish an empathic but bounded relationship on the basis of understanding the client and promote mentalization. Techniques such as free association and empathic interpretation are used to increase the client's awareness and reflection, and to accompany the client steadily, promoting the completion of symbiosis, separation and individuation. At present, the client can have a better understanding of how the past has hijacked the current feelings, can have a better relationship with boundaries,and can better understand and accept themselves.
