认知换框干预阈下抑郁大学生的一例个案研究

  • Abstract: This case study examined the effects of a five-week cognitive reframing intervention on a second-year female graduate student with subthreshold depression. The intervention was structured around a “Because X, therefore Y” sentence format, which aimed to systematically modify negative causal interpretations. Through this structured process, the client was guided from a predominantly negative attributional style to a more flexible and multidimensional explanatory framework. Quantitative results showed that the client’s Beck Depression Inventory-II (BDI-II) score decreased from 20 at baseline to 0 at postintervention, and her Center for Epidemiologic Studies Depression Scale (CES-D) score decreased from 27 to 3. These improvements were maintained during the two-month follow-up assessment. Qualitative findings further suggested reductions in anxiety and psychological distress, increased cognitive flexibility, partial modification of perfectionistic tendencies, and generalization of cognitive reframing skills to academic, interpersonal, and daily life contexts. The findings provide preliminary casebased evidence for the potential utility of cognitive reframing in university students with subthreshold depression. 本案例以一名阈下抑郁的研二在校女生为对象,开展为期5周的认知换框咨询。咨询以结构化“因为X,所以Y”句式为核心,通过系统性修正负性因果解释逻辑,引导来访者从单一负性归因转向多维解释框架,逐步帮助其跳脱出固有的“思维之框”。量化结果显示,来访者BDI-Ⅱ得分由基线期20分降至干预结束时0分,CES-D得分由27分降至3分,且在2个月追踪期内保持稳定。质性资料表明,来访者焦虑与痛苦情绪显著改善,认知灵活性增强,完美主义倾向得到一定程度修正,并能够将认知换框技术迁移至学业、人际及生活事件。案例结果显示,认知换框技术对阈下抑郁大学生具有积极干预价值,为该技术在高校心理健康服务中的应用提供了初步个案证据。
