A Case Study on Integrated Intervention of Family System and Trauma Therapy — Intergenerational Bondage and Resilience Reconstruction of a Female Client with Multiple Traumas

Abstract:
This case report presents a counseling practice integrating family systems therapy with trauma-focused therapy. The client was a female who presented with complex post-traumatic stress disorder (CPTSD) accompanied by chronic low back pain as a somatic symptom and social dysfunction, stemming from traumatic childhood sexual experiences, secondary trauma in adulthood, and intergenerational family pressure. The counseling followed the three-phase model of trauma therapy, employing genogram analysis to examine intergenerational patterns, along with interventions such as stabilization techniques, the pendulum approach, and family sculpture over 12 structured sessions. Following the intervention, the client’s somatic symptoms were significantly alleviated, social functioning gradually recovered, and she successfully established a healthy intimate relationship, achieving an identity reconstruction from “family responsibility bearer” to “independent individual”. This case provides a phased, replicable practical reference for interventions addressing the interplay between multiple traumas and family systems, as well as practical insights for counseling clients with coexisting trauma and family issues in university settings.
本案例报告展示了一例整合家庭系统治疗与创伤疗法的心理咨询实践。来访者为女性,受童年创伤性性经历、成年期二次创伤及家庭代际压力影响,呈现复杂性创伤后应激障碍(CPTSD),伴随慢性腰痛躯体化症状及社交功能障碍。咨询遵循创伤治疗三阶段模型,运用家谱图分析代际互动模式,结合稳定化、钟摆技术及家庭雕塑等干预方法,开展12次结构化咨询。干预后,来访者躯体症状显著缓解,社会功能逐步恢复,成功建立健康亲密关系,完成从“家族责任承担者”到“独立个体”的身份重构。本案例针对多重创伤与家庭系统相互影响的问题开展分阶段干预,具备可复制的实践参考价值,也为高校中存在同类创伤与家庭问题的来访者心理咨询提供实操思路。
